To apply for 100 Club membership you need to:

a) Click on the link below and print off and complete the form and email it to Dean McShane:dean.mcshane@ntlworld.com

b) Then, set up a Standing Order from your bank account (via internet/telephone banking). 
- Make sure you make your entry identifiable by making your name the reference (i.e J.SMITH.100).
